All contributions are VOLUNTARY & confidential; helping MCOFA to expand programs to others. NO Service will ever be denied for inability or unwillingness to make a contribution.

• Equipment from the Loan Closet
• $5.00 for small items (i.e. walkers, grab-bars, commodes, shower seats)
• $20.00 for larger items (i.e. transport chairs, wheelchairs, transfer benches)
• Caregiver Respite - $3.00 per session with Case Manager
• EISEP - $3.00 per session with Case Manager
• File of Life Magnetic Pocket - $3.00 per pocket
• Handyman/Home Helper list - $2.00 per list
• Health Insurance Counseling - $3.00 per session with HIICAP Counselor
• Legal Services - $10.00 per referral to Legal Aid
• Meals
• Congregate Meal - $3.50 per meal
• Frozen - $3.50 per meal
• Home Delivered Meal - $3.95 per meal
• Nutrition Counseling - $10.00 per session with Registered Dietitian
• Office Visit or Information - $2.00 per session with staff member
• Senior Discount Card - $3.00 per card
• To be placed on Home Helper/Handyman list - $10.00
• Transportation Program
• $ 4.00 rides 0-10 miles
• $ 8.00 rides 10-30 miles
• $ 12.00 rides 30-50 miles
• $ 16.00 rides over 50 miles
